Important: The hiring manager will now complete all sections of the form including the ethnicity, race and sex columns based on visual observation.
- There are new codes for completing those columns listed on the second page of the form. (In the interest of going green, it is not necessary for you to print that page.) The new codes match the codes in PennWorks.
- PennWorks requires two entries for Race and Ethnicity. First the question of ethnicity must be answered (Hispanic? Yes or No), and then of race (employee may identify as many as apply). If you have not already experienced this, you will the next time you enter a new employee into PennWorks who was hired from outside Penn. Please have the employee complete the new Voluntary Self ID form (http://www.upenn.edu/affirm-action/VolSelf_ID_Form.pdf) to capture this information so you can enter it into PennWorks accurately. I recommend sending it with the offer letter. Please note that the employee can self-identify as many race selections as apply. PennWorks will accept multiple entries. Please forward the Voluntary Self ID form to Wharton HR with the copy of the offer letter. You do not need to keep it in the department. Michelle will include both the ethnicity and the race codes that you will need for PennWorks on the profile sheet.
